Direct Entry Box

For Direct Entry shipments, please be sure to check the Direct Entry Box under Added Services and select appropriate product and service level.

Commodity Shipped

In order to meet FDA requirements, shipper must check "Food" box if sending food products including vitamins and nutritional supplements. For all other commodities, shipper must check "Others" box and provide a general description identifying the product(s) such as clothing, books, computer parts, etc.

Shipment Information

Must be completed if the shipper would like to insure the shipment. A declared value must be noted. Please contact Client Services for details and pricing.

Mail Pieces, Containers, Weight (required)

Identify the number of pieces, containers, and weight (lbs.) of the shipment.

BOL Number

The Bill of Lading number.

Order Confirmation

Please record the Order Confirmation Number that was provided by your DHL Global Mail Customer Service Representative when you scheduled your pick-up.

Billing Reference (optional)

Identify any internal reference numbers.

Shipment Authorization Field (required)

To be completed by the shipper and signed.

Pick-up Agent

To be completed by DHL Global Mail and its agents.

Peel-Off Labels

Labels can be placed on each bag or box of multi-container shipments for identification.
